GROUP TREATMENT
INTAKE
ASSESSMENT
The intake assessment is utilized to gather and document a history of
present illness. This process evaluates the client's status to determine
whether any of the following conditions exist: intoxication, withdrawal or
related medical problems, psychiatric conditions, disorders affecting social
stability and other similarly causative conditions. The intake gathers a
complete substance abuse history including: substances used in the past,
substances used recently, substances of preference, frequency and amount of
each substance, any previous occurrences of overdose, withdrawal or average
drug and alcohol reactions, history and year(s) of all previous substance
abuse treatment, complete medical check list, complete T.B. assessment,
complete HIV Risk Assessment, a social history and a complete mental status
examination. The intake assessment leads to an integrated clinical
formulation.
During the interview intake process: treatment modules, treatment tracks,
policies, procedures and fees are fully explained to each client. A
signature of fee agreement and consent are secured during this process. Each
client seeking treatment is provided with a recipient rights statement and
HIPPA documentation that is signed by the client. The client is made aware
of applicable program rules and hours of operation. If a client is indigent
or meets OCHD/OSAS household income requirements, OCHD/OSAS may be able to
completely or partially subsidize treatment. OCHD/OSAS admission data is
obtained from the client. If the client is income eligible, the client and
therapist sign a declaration of income attesting to income level. Clients
with personal income at a subsidy level and a household income above the
level may be eligible for subsidy if the household income is unavailable for
treatment assistance. In those specific cases, a declaration of income
adjustment form will be submitted to OCHD/OSAS. A tentative plan is
developed from the clinical formulation and the client referred to the
appropriate treatment track.
HISTORY AND
PHYSICAL EXAMINATION
The medical history and physical examination is utilized to determine if the
client is capable of responding to treatment in an outpatient environment.
During this process, the physician conducts a medical evaluation that
includes the following: a mental status examination, a substance abuse
history, a determination of the client's date of last use of alcohol and/or
drugs and a physical examination. The data gathered through this process,
along with the physician's review of the intake assessment, allows the
physician to determine if the client requires medical detoxification and the
appropriate level of are. At this time the physician may order laboratory
tests and/or prescribe medication as are needed. The Counseling Center
presently has a service agreement with an outside laboratory to provide a
full range of medical tests including drug screens, screening for sexually
transmitted diseases, pregnancy testing, and pre-prescription work-ups.
PHYSICIAN
FOLLOW-UP
The physician to meet with the client to review laboratory findings and/or
review medication(s) prescribed utilizes the physician follow up.
INDIVIDUAL
THERAPY
Individual therapy will be utilized to provide the client with continued
assessment, to assist the client in the development of an individualized
treatment plan, to provide ongoing evaluation of the treatment process, to
offer an opportunity for client to work on issues which may not be
appropriate for discussion in group therapy. Individual therapy will be
utilized to assist the client in developing a recovery plan, which will
assist him/her in developing a lifestyle that is consistent with recovery.
This modality of treatment is used to challenge denial systems and
facilitate movement toward acceptance. In addition, individual therapy will
be used to reinforce and emphasize positive behaviors and to challenge
cognitive distortions that are inconsistent with recovery.
Interventions: Cognitive behavioral, person centered and humanistic.
FAMILY COUNSELING
Family therapy is utilized to treat a family in which chemical dependency is
a presenting problem and at least one family member has been diagnosed as
chemical dependent. This form of treatment addresses the effects that
addiction has on both the addicted and non-addicted (co-dependent) members
of the family and is aimed at treating addiction as a "family disease."
Interventions: Education, circular questioning,
family sculpturing, role-playing, directives and homework assignments.
PSYCHIATRIC
EVALUATION
Psychiatric evaluation is utilized to provide a vehicle by which the
psychiatrist can both evaluated and provide treatment and supervision in the
client's treatment. The psychiatric evaluation will consist of the
following: mental status examination, history of present illness,
psychosocial assessment, treatment recommendation, medication recommendation
and the prescription of medication when appropriate.
MEDICATION
REVIEW
The psychiatrist to review the clients medication utilizes the medication
review. This service allows the psychiatrist to monitor the client's
response to the medication prescribed, evaluate the effectiveness of the
medication, adjust dosage as is appropriate, change medication when needed
and monitor for side effects.
GROUP TREATMENT
Group therapy is the process oriented treatment group in which clients enter
group in order to work on their issues related to recovery. Group therapy is
utilized as a mechanism to foster group support as well as peer group
confrontation. This modality is used to assist clients in developing coping
skills without the use of mood altering chemicals, to facilitate their
development of adequate problem solving skills, to resolve grief issues, to
enhance self-concept and to increase self esteem for sobriety maintenance.
In addition, group therapy is used to assist clients in adopting values and
belief systems consistent with recovery. The overall goal of group treatment
is to assist the client in developing quality sobriety.
Interventions: Cognitive behavioral, positive reinforcement, encounter,
empathy skills and modeling. |
